Empowering individuals to lead meaningful lives, the Case Management and Psychosocial Rehabilitation (PSR) teams work hand-in-hand to provide therapeutic services for adults facing life’s most challenging stressors.
Their focus is on equipping individuals with the social, emotional, and intellectual tools needed to thrive, especially those living with severe mental health conditions such as schizophrenia, bipolar disorder, borderline personality disorder, and major depressive disorder.
For many of these individuals, mental illness has profoundly impacted their daily lives. Struggles with maintaining employment, managing finances, attending to personal hygiene, and building a healthy support system are common. They may also navigate complex systems to access resources, making the support of these teams critical.
How the Teams Help
- Therapists provide individual, group, and family therapy to address emotional well-being and improve functioning.
- Case managers connect clients to vital community resources and offer skill-building education, motivational interviewing, and tools for symptom management to foster independence.

- In 2024, these teams served 1,523 unique clients and provided 16,266 services to individuals in need.
- Not all services are covered: 5% of therapy services and 30% of case management services were provided without reimbursement.
